We won’t make you jump through hoops (that’s just for the dogs on our agility course), but this form is a little longer than most. We want to get to know your dog properly, from personality to routine, so we can make sure Urban Mutts is the right fit for them and they’re the right fit for us.

Required for hotel and The Whole Pack-age.
This person must live within two hours of Urban Mutts and be available in case of emergency. Please don’t list yourself again - this is a licensing requirement.

Consents & Policies

Please take a moment to review and confirm the following. These help us provide safe, personalised care that meets licensing standards.

Vaccination Policy
In line with our animal welfare policy, all dogs must be fully vaccinated before joining Urban Mutts.


Required vaccinations:

  • Leptospirosis (L2 or L4
  • DHP (Distemper, Hepatitis, Parvovirus)
  • Kennel Cough (recommended)

Behaviour Policy
If a dog shows anti-social or unsafe behaviour, they’ll be cared for one-to-one while the situation is reviewed. We’ll work with you to create a plan, or assess whether Urban Mutts is the right fit.

Care & Activity Consents
Please let us know what you’re happy for your dog to take part in:

Do you give consent for your dog to be walked with other dogs?

Do you give consent for your dog to mix with dogs from other households in outdoor spaces?

Do you give consent for your dog to share a sleep hut with other dogs from your household?

Socialising with a range of ages is a core part of life at Urban Mutts. We can’t guarantee there will always be other puppies under 1 on site, and we don’t offer age-restricted groups. If you select “No”, we may not be able to accommodate your booking, as our 1:6 staff-to-dog ratio means one team member cannot supervise a single dog all day.

Do you give consent for your dog to be walked off lead in a safe park or enclosed facility?

Do you give consent for us to use a crate if needed for rest or safety?

Do you give consent for us to contact one of our registered veterinary partners if your dog requires treatment during their stay?

We work with two trusted veterinary partners:

  • Palace Vets – our on-site vet, used for minor illnesses, checks, or non-urgent diagnostics
  • Veteris – a full-service hospital we use for surgical needs or more serious medical cases

If your dog becomes unwell or injured and you're not reachable, we may need to act quickly. We will use the most appropriate provider based on the situation. Additional fees will apply, and you will be responsible for any treatment costs.

Do you give consent for us to treat your dog for external parasites (fleas, ticks, lice) and cover the cost if needed?

If you select “No”, your dog will be placed in isolation and must be collected within two hours by you, your secondary contact, or your emergency contact. For Daycare, we can usually work around this. For Hotel stays, this may not be manageable. This policy is not negotiable - your emergency contact must be able to collect. We strongly recommend saying yes to this.
